These forms are designed to make recording an accident or injury in school quick, clear and simple. They have been developed using feedback from teachers and first aiders, providing an excellent starting point. In addition to incorporating your school name, logo, and contact details, they can be amended to your specifications. How to edit a template or create a new layout
Our pre-made accident and injury form templates are often a great fit for schools, but they can also be customised to fit your school’s specific needs. If you prefer, we can create unique first-aid forms based on your artwork or instructions.
When you fill out the online order form, you’ll be guided through the following process. Start by choosing the template you want to modify. Then, you can provide a list of instructions or attach files that describe the changes you need. We will make the necessary adjustments and send proof artwork for your approval before printing. You can request further changes at this stage, and we will continue to provide proof artwork until you are completely satisfied with your school accident books.
If you’re currently using your own accident and injury forms, or if you want us to design a new layout based on your current system, simply attach your files and/or provide instructions when completing the online order form.
